As a courtesy to our patients, we will accept assignment of your insurance benefits. We will verify your insurance coverage after you have supplied us with a copy of your insurance card.
Unlike medical insurance, orthodontic insurance does not usually cover 100%. A convenient payment plan can be arranged for the balance of your treatment fee.
It is not necessary to supply us with an insurance form.
